Appendix 3: Sample constituent outreach log
Below is a log that can collect and collate information
from town halls and used to track follow up activities. This
is distinct from the sign-in sheet found in Appendix 5 be-
cause this sheet is used to track specific issues raised where-
as the sign-in sheet just looks to track attendance.
If desired, this sheet need not record names or contact in-
formation unless constituents specifically request to be con-
tacted again with additional information.
